    If possible, all entries from a team or school should be submitted together.

Eligibility Certification

This certification must be signed by a school official / teacher accompanying the team.  If no school official/teacher is attending the event, the official team coach may sign on behalf of the school.

I hereby certify that the team members entered in the 2003 U.S. Interscholastic Orienteering Championship from the above named school meet the criteria set forth in the USOF Rules of Competition, including:

1.    All members of the team are currently enrolled in the above-named school.
2.    for Varsity Teams: Are full-time high school students, 9th - 12th grade, and age 19 years or younger as of 12/31/2004.
3.    for Junior Varsity Teams: Are full-time high school students, 9th - 12th grade, and age 17 years or younger as of 12/31/2004.
4.    for Middle School Teams: Are full-time students, 9th grade or below, and age 16 years or younger as of 12/31/2004.
5.    for JROTC Teams: Are enrolled in the same JROTC unit per the most current Unit Enrollment Report.
6.    All team members are USOF members in good standing.
